Conviva Senior Primary Care is a true partner in your health.

Our providers and team members take the time to get to know you and provide the personalized care that’s right for you—today and into the future.

The Medicare Annual Enrollment Period (AEP) is almost here. AEP runs from October 15 to December 7, and is your chance to review your current Medicare coverage to ensure you have the best plan for you.

Keep the doctor you trust

If you’ve received your Annual Notice of Change and are considering your Medicare options, your Conviva team can help you complete a Medicare Annual Enrollment checklist, which helps you identify what matters most to you when it comes to your healthcare. This can make it easier to compare plans and choose the one that’s right for you.

Make 2025 your best year yet!

Once your AEP decision is final, don't forget to schedule your Annual Wellness Visit in 2025.

Did you know that coming in for a wellness visit helps us to identify health risks and allows us to develop preventative care plans that are personalized to you? We spend 50% more time with our patients*, which allows us to discuss your health and so much more. Your AWV typically includes:

  • Checking your blood pressure, weight, vision, and hearing to identify and schedule any needed routine health screenings, such as diabetes and cancer screenings. These preventative health screenings are critical to keeping you out of the hospital and in the best health possible.
  • Going over your health history.
  • Discussing your health goals and address any concerns.
  • Reviewing your medications.

Our goal is to keep you well, and your no-cost Annual Wellness Visit is the first step in doing just that. Schedule your Annual Wellness Visit by calling your center today.
