Virtual primary care is like a doctor’s appointment you would receive in one of our centers, except it is completed online via computer or smart phone.

 

You will have a dedicated care team that gets to know you over time, helps you manage chronic conditions, collaborates with you on preventative care, and can refer you to specialists as needed.

 

All program participants have access to essential medical devices in the home such as a blood pressure cuff, weight scale, thermometer, and pulse oximeter. These devices will help inform your physician about next steps for care. If an in-person visit is needed, our team will schedule an appointment at the closest center or have a care team come to your home. 

Because Conviva Primary Care Anywhere has both virtual and in-center capabilities, we can handle any primary care needs. You can expect the same services you currently receive from your primary care provider, plus additional options such as secure messaging and same-day video visits.

 

Our service offerings include routine wellness care, early detection and screening, labs & diagnostics, disease management programs, hospital care coordination, specialist referrals, advanced care planning, care at home, and social services. We also offer same-day appointments to ensure that you can get timely care.

A smart phone or laptop is all you need to access video appointments and your online patient information. You can join a video visit from our mobile app, our online patient portal, or directly from any web browser.

Our Care Team will troubleshoot any technical issues that occur during an appointment. For technical issues that occur outside of an appointment, you can call our phone line at 561-782-7686 and press 1.

Yes, you will always be able to reach a member of the Conviva team with questions or concerns. During office hours, you can reach our front office by calling 561-782-7686 and pressing 1 for "existing patient". After business hours, calling the same phone line will connect you to an on-call physician or nurse practitioner, who will support you with any questions or issues that can't wait until the next morning.

Yes. We hire our own staff of board-certified, state-licensed doctors and other qualified care providers. When you sign up for Conviva Primary Care Anywhere, you will be matched with a dedicated care team with expertise in senior care (physician and Care Partner).

You will not be able to keep your existing primary care physician as our program's physician will be assigned to you as your new dedicated PCP. However, you can keep any specialists you have previously seen. You do not need to inform your current physician in advance. If we accept you into the program, our team will handle all the patient transfer logistics.

If you have an issue that requires in-person care, your Care Partner will help to schedule your appointments to ensure seamless coordination and follow-up. They will continue to be available for same-day televisits and conversations over secure messaging to make sure you feel supported.

 

Your care team will also help you connect with the right specialists, pharmacies, and diagnostics labs. 

When you need help from a specialist or require inpatient treatment, we will refer you to providers we trust, and your Care Partner will follow up with you every step of the way to ensure that your needs are being met.

 

We can help coordinate labs and imaging at your closest Conviva Care Center or community partner. Additionally, Conviva Primary Care Anywhere can schedule some labs and diagnostics in your home, depending on what is in-network with your insurance plan.

The service is currently available in the Palm Beach and Treasure Coast areas. 

We work with health insurance plans to offer Conviva Primary Care Anywhere at no added cost to Medicare patients. Depending on your health plan, you may still have a standard copay for physician's visits, which would remain unchanged in this program. Any interactions with your care team that are resolved via secure messaging or phone call will have zero copays. You can communicate with your care team as much as you like.

Conviva Primary Care Anywhere services are covered under these Medicare plans with no cost to you. As we expand coverage, be sure to review periodically for updates.
