You should have received a letter asking you to review and confirm your contact information. If any of it is incorrect, call your local Conviva Senior Primary Care center to update your contact info.
You can continue using the existing patient portal until your Conviva Senior Primary Care center transitions over to the new portal powered by athenahealth®.
The new Conviva Patient Portal will begin rolling out at the end of June 2025. This will be a phased approach based on your center location. Once it has launched, you will receive a link to register.
A patient portal is a website that provides direct access to the medical information recorded in your patient chart. Many patient portals support additional features, including the ability to pay bills or message physicians or care teams directly.
Our Patient Portal powered by athenahealth® is a secure tool that allows you to:
View upcoming appointments
Communicate and message with your doctor and care team
Review referral history and lab results
Request medication refills
Access visit summaries and personal health record
Update your personal information
Connect with your care team from home with a TeleVisit, chat functionality available
Share access with a caregiver*
Conviva is updating the way we manage medical records online. You will be able to communicate better with your doctors and have all the tools you need in one place. This change shows our commitment to providing you with the best care possible.
You can continue using the existing patient portal until your center transitions over to the new portal powered by athenahealth®. You will need to register for the new portal at that time.
