Dr. Subhadra Chereddy joined Conviva Care Center, Middleburg, FL as a board-certified family medicine physician specializing in geriatrics where she leads a team of compassionate, patient-focused primary care professionals with a dedication to healthcare excellence. She completed her Family Medicine Residency from the University of Minnesota North Memorial Residency program in 2011. Before joining Conviva Care Center in 2023, Dr. Chereddy worked as an attending physician at multiple skilled nursing facilities with Twin Cities Physicians in Minneapolis, MN. Earlier, she worked as a primary care physician with Hennepin Healthcare and Mayo Health Systems. Dr. Chereddy focuses on her patients well-being and independence based on their wishes and expectations.
