You may be notified by your health plan that your provider along with Conviva will no longer be a provider in your health plan and will be considered out-of-network. If you are not on Medicare, you may contact the number on your insurance card for information about other doctors available in your health plan’s network.

If you do not see your plan listed above, you or your new provider may request a copy of your medical records. Existing patients records will remain at the clinic. Please return this completed consent form to the front desk staff and they will assist you.
