Mrs. Diana Joan Hernandez, MSN, AGPCNP-C, is an Adult-Gerontology Primary Care Nurse Practitioner dedicated to providing compassionate, patient-centered care. She earned her Master of Science in Nursing from the University of Texas at Arlington in Arlington, Texas. Mrs. Hernandez worked in family practice, endocrinology, palliative care and long-term care settings, caring for adults and older adults with complex and chronic health conditions. Her experience included managing diabetes and endocrine disorders, conducting palliative care assessments and supporting patients through transitions of care. She also served in leadership roles overseeing clinical operations, staff development, workflow improvement and patient engagement initiatives across multiple healthcare settings. Guided by her belief that listening to patients is essential to improving health and quality of life, she is especially passionate about diabetes management, nutrition and addressing food insecurity. Outside of work, she enjoys spending time with family, enjoying the outdoors and caring for her plants.
