Eva Jacques, DNP, APRN, joined Conviva as a doctorally prepared, board certified nurse practitioner. Ms. Jacques earned her Doctor of Nursing Practice from Grand Canyon University, her Master of Science in Nursing from Florida International University, and her ASN from Miami-Dade Community College. Before joining Conviva in 2022, Ms. Jacques was a nurse practitioner at CareMax Medical Centers in Davie, FL, providing primary care to older adults. She also worked as a primary care nurse practitioner with Fatima Medical Center and as a primary care provider with New Health Community Centers in Miami, FL. These and earlier positions have contributed to her experience and expertise in caring for seniors. She values the collegial atmosphere and spirit of cooperation at Conviva. Ms. Jacques entered the healthcare field because of her commitment to helping others. She believes in providing individualized care by getting to know her patients and their families. She also ensures patient satisfaction by listening to their concerns. It makes her happy to form genuine bonds with her patients and see them achieve optimal health and well-being. In her free time, she enjoys attending concerts.
