Heber J. Rosa, M.D., is board-certified through the American Board of Family Medicine, and has been providing compassionate, patient-focused primary care in the Jacksonville community since 1996 when he completed the Family Medicine Residency Program at the University of Florida, Jacksonville, and served as Chief Resident. Dr. Rosa has been with MCCI/Conviva since 2017. He earned his medical degree from the Universidad Central del Caribe in Bayamon, Puerto Rico in 1993, as well as a Bachelor of Science from the University of Puerto Rico, Rio Piedras, and is a member of the Alpha Omega Alpha Medical Honor Society. Dr. Rosa has been recognized through the NCQAs Diabetes Recognition Program (DRP). Clinicians who achieve DRP Recognition show their peers, patients and others in the Diabetes community that they are part of an elite group that is publicly recognized for its skill in providing the highest-level diabetes care. Dr. Rosa enjoys photography and music.
