Ms. Rachael Ann Hammel is a board-certified Family Nurse Practitioner with a strong focus on geriatric care and chronic disease management. She earned her Master of Science in Nursing from Harding University and holds board certification through the American Academy of Nurse Practitioners. Currently practicing at Oak Street Health in Charlotte, NC, Ms. Hammel provides comprehensive care to Medicare and Medicaid patients, specializing in managing complex comorbidities in seniors through a value-based care model. Her previous roles include primary care at Westmoreland Family Clinic and teaching undergraduate nursing students as adjunct faculty at Aspen University. With a background in pediatric and medical-surgical nursing at Vanderbilt University Medical Center and Cabell Huntington Hospital, she brings a well-rounded clinical perspective. Outside of work, Ms. Hammel enjoys cooking and the outdoors.
