Mr. Rene Leyva is a Family Nurse Practitioner with extensive experience in primary care, geriatrics, chronic disease management, and community health across outpatient clinics, home health, rehabilitation centers, and academic settings. He brings a unique dual background as both a former physician in Cuba—serving as a Medical Director in critical care, family medicine, and geriatric settings—and as an experienced nurse practitioner in the United States. His career includes roles in home health care, federal health services, public health, and clinical education, including serving as a preceptor for nurse practitioner students. He is dedicated to assessing complex medical needs, supporting chronic condition management, and helping patients prioritize their health through compassionate, patient centered care. Guided by a philosophy of caring for people regardless of age or circumstance, he is passionate about diabetes, heart and brain health, exercise, social connection, and nutrition. Outside of work, he enjoys cooking, reading, and spending time with his family.
