Yazmin Valencia, DNP, FNP-BC, has joined Conviva as a doctorally prepared, board certified family nurse practitioner. Ms. Valencia earned her Doctor of Nursing Practice and her Bachelor of Science in Nursing from the University of the Incarnate Word in San Antonio, Texas. Her accolades include NCQA Recognition for Diabetes and Heart/Stroke Care in 2021. Before joining Conviva in 2023, Ms. Valencia was a family nurse practitioner with Cano Health in San Antonio and, previously, Rocha Medical Clinic in San Antonio and Somerset, Texas. She practiced as a registered nurse at Connally Memorial Hospital in pre-operative and post-anesthesia care, and earlier at Metropolitan Methodist Hospital in surgical intensive care and Warm Springs Specialty Hospital in a high observation unit/wound care unit. Passionate about addressing diabetes, heart health, and food insecurity, she believes in empowering her older adult patients through comprehensive, accurate information. This philosophy supports Conviva's commitment to patient-centered care. Ms. Valencia chose healthcare to help others and serve the community. Outside of work, she enjoys the outdoors, spending time with family, and traveling.
