Conviva Senior Primary Care offers senior-focused primary care with physicians supported by a dedicated Care Team. This allows us to provide our patients proactive care, no-rush appointments, same-day appointments and timely follow-ups. Read more about our services and how we’re different.

To become a patient, you can complete this form on our website and a team member will reach out to help and answer any questions you may have. You can also call or visit a Conviva Senior Primary Care location near you, where you will receive a new patient information packet that you can bring to your first appointment. If your medical records are with another physician, we will ask you to complete a Medical Records Release form that gives your new Conviva Senior Primary Care doctor permission to request the records.

Arrive early to your first appointment to fill out your new patient paperwork.

In addition, you should bring:

- Your Photo ID (such as a driver’s license or a passport)
- Your insurance card
- A list of your current medications

Please call 833-220-8654 to schedule an appointment. Same-day appointments are usually available in case of an urgent need. To schedule lab appointments, please call us in advance to avoid longer wait times.

Conviva Senior Primary Care patients have the option to see their doctor remotely with a virtual visit. These visits are private and secure, allowing you to connect one-on-one with your provider from home or wherever you’re most comfortable. Virtual visits can be accessed through the patient portal or a link sent by text or email.

We accept a variety of insurance plans that vary by location. To learn more about the plans we accept, including how to switch to an accepted plan, please visit our location finder. To change doctors, you can call 833-220-8654 and one of our care team members can guide you through the process. The most important things to consider are whether the potential new doctor is accepting new patients, whether they accept your insurance, and whether you feel comfortable with them.
